Dina Bharier provides nutritional therapy services to clients to improve their health through diet and lifestyle changes. At Nutri Flourish the focus is on preventative healthcare, the optimisation of physical and mental health, and chronic health conditions. Through nutritional therapy consultations, and dietary and lifestyle analysis. Dina Bharier aims to understand the underlying causes of your health issues and will seek for those to be addressed through personalised kosher nutrition, and supplement and lifestyle advice.

HOW WE OBTAIN YOUR PERSONAL DATA:
You provide Dina Bharier with personal data in the following ways:

  • By filling out a nutritional therapy questionnaire

  • By signing a term of engagement form

  • Via email or by phone

  • By taking online payment

The details taken will include the following:

  • Basic details such as name, address, contact details and next of kin

  • Health information including your previous medical history, dietary, lifestyle, supplement and medicine details

  • GP contact information

Dina Bharier uses this information in order to provide you with direct healthcare. This means that the legal basis for holding your personal data is for legitimate interest.

Following completion of your healthcare your personal data is kept for the period defined by Dina’s professional association BANT and registration body CNHC. This enables Dina Bharier to process any complaint you may make. In this case the legal basis of holding your personal data is for contract administration.

Dina Bharier may keep Sensitive information from other healthcare providers. The provision of this information is subject to you giving Dina Bharier your consent. If Dina Bharier does not receive your consent, coordinating your healthcare with that provided by other providers will not be possible. Which may affect the healthcare provided by Dina Bharier.

Dina Bharier acts as a data controller for use of your personal data to provide direct healthcare, as well as processing online payments.

Dina Bharier prioritises protecting your personal data at all times, including any health and contact details in a professional manner, and according to the requirements of the General Data Protection Regulation (GDPR) and also takes reasonable security measures to protect your personal data storage.

Dina Bharier may use your data for marketing purposes such as newsletters but this would be subject to you giving us your consent.

Dina Bharier will ask for your consent before sharing your information with your GP or any other healthcare providers. However if is there is reason to believe that your life is in danger then we may pass your information onto an appropriate authority e.g police, or your GP.

HOW LONG DO YOU HOLD CONFIDENTIAL INFORMATION FOR?
All records held by the Dina Bharier will be kept in line with our records retention policy. They also take into account any legal, statutory and regulatory obligations. Our need to use your personal information will be reassessed on a regular basis and information which is no longer required will be disposed of.

Dina Bharier uses electronic forms on their website which has a number of built-in features to help ensure privacy. Nutri Flourish also aims to use secure forms where appropriate.

Dina Bharier uses cookies to collect information about how visitors use their site. The information is used to compile reports and to help improve the site. The cookies collect information in an anonymous form, including the number of visitors to the site, where visitors have come to the site from and the pages they visited.

Like most websites, Nutri Flourish makes use of analytics software in order to help them understand the trends in popularity of their website and of different sections. There is no use made of personally identifiable information in any of the statistical reports that is used from this package.
